About Us
Community Health Network is a Federally Qualified Health Center with 14 locations throughout Brazoria, Galveston, and Southeast Harris counties. Our mission is to provide quality care that uplifts, supports, and shapes healthy communities while addressing financial and social barriers. In our health centers, we provide a variety of services such as primary care, pediatrics, women’s health, behavioral health, dental, pharmacy, and eligibility services.
